Herrera Beutler delivers prematurely, baby OK
Source: The Columbian, Vancouver, Wash. July 29--Congresswoman Jaime Herrera Beutler delivered her baby earlier this month, and doctors are considering the baby to likely be the first to survive a diagnosis previously considered fatal.Herrera Beutler and her husband, Daniel Beutler, welcomed their daughter, Abigail Rose Beutler, at 3:13 a.m. July 15 at Legacy Emanuel Medical Center in Portland. Abigail weighed 2 pounds 12 ounces at birth. While in utero, Abigail was diagnosed with Potter's Syndrome, a condition associated with a deficiency in amniotic fluid often caused by a baby's prenatal kidney failure. In Abigail's case, absence of the kidneys resulted in a complete absence of amniotic fluid in the womb, which is critical to lung development, according to a news release from Herrera Beutler's office.Babies born with the condition typically suffer from severely underdeveloped lungs and are unable to breathe once outside of the womb.Multiple doctors told Herrera Beutler and her husband that, based on medical evidence, Abigail's condition was incompatible with life and if she survived to term, she would be unable to breathe and live only moments after birth."The options we were offered were termination or 'expectant management,' that is, waiting for her to die," Herrera Beutler said in a Facebook announcement. "Instead, we chose to pray earnestly for a miracle."Delivered pre-term at 28-weeks gestation -- full term pregnancy is 38 to 40 weeks -- Abigail is believed to be the first baby on record with the condition to breathe sustainably on her own, according to the news release.Herrera Beutler, 34, received the diagnosis during her second trimester of pregnancy. Upon learning the news, she and her husband found a team of specialized physicians at The Johns Hopkins Hospital in Baltimore. There, Herrera Beutler underwent serial amnioinfusion, a relatively uncommon treatment in which a saline solution is injected into the womb in the place of amniotic fluid."Through the outreach of a parent who had faced a similar situation, we found a group of courageous and hopeful doctors at Johns Hopkins who were willing to try a simple, but unproven treatment by amnioinfusion," Herrera Beutler and her husband said in the news release."Every week for five weeks, doctors injected saline into the womb to give Abigail's lungs a chance to develop. With each infusion we watched via ultrasound as Abigail responded to the fluid by moving, swallowing and 'practice breathing.' The initial lack of fluid in the womb caused pressure on her head and chest, but over the course of the treatment we were able to watch them reform to their proper size and shape. Her feet, which were clubbed in early ultrasounds, straightened," they said. "There was no way to know if this treatment would be effective or to track lung development, but with hearts full of hope, we put our trust in the Lord and continued to p儲存ay for a miracle."After the fifth infusion, Herrera Beutler went into pre-term labor while at home in Washington. Abigail was delivered four days later with fully developed lungs, according to the news release.Without functioning kidneys, Abigail was still critically at risk, however, and required immediate peritoneal dialysis under intensive care. At 16 hours old, Abigail was transported to Lucile Packard Children's Hospital at Stanford University in Palo Alto, Calif. She had surgery the next day and began dialysis immediately after, according to the news release."The first doctors we encountered told us that dialysis or transplant were not possible," Herrera Beutler and her husband said. "No local hospital was prepared to perform the procedure necessary -- peritoneal dialysis -- on a baby so small. Johns Hopkins was willing, but too far to transport. After a day of searching, we found a team at Lucile Packard Children's Hospital who were 'happy to have her.'""Despite Abigail's prematurity, small size and life-threatening disease, she is doing well," said Dr. Louis Halamek, neonatologist at Packard Children's and professor of neonatal and developmental medicine at the Stanford School of Medicine. "Together with our colleagues in pediatric nephrology, we are cautiously optimistic about her future.""This case is unprecedented," said Dr. Jessica Bienstock, a professor of maternal-fetal medicine in the Department of Gynecology and Obstetrics at the Johns Hopkins University School of Medicine who led the Hopkins team that performed the serial amnioinfusions. "It would be premature to say bilateral renal agenesis should always be treated using serial amnioinfusion, but this suggests it can be part of the conversation when that is the diagnosis. Hopefully science will evolve to the point where we will be able to save babies with this defect. But so far this is just one isolated case whose ultimate outcome is still unknown."Abigail is currently active, stable and breathing on her own at Lucile Packard Children's Hospital. Abigail will require ongoing dialysis and an eventual transplant."We are grateful to the doctors and nurses in Baltimore, Vancouver, Portland and California who, like us, were not willing to accept the fatal diagnosis, but were willing to fight for the impossible," Herrera Beutler and her husband said. "We are grateful to the thousands who joined us in praying for a miracle. But most of all, we are grateful to God for hearing those prayers."For more information on Abigail's specific case, to access photos of the family at Packard Children's or to see full statements from Herrera Beutler and Buetler, The Johns Hopkins Hospital and Lucile Packard Children's Hospital, visit .crosbyvolmer.com/jhbeutler.Copyright: ___ (c)2013 The Columbian (Vancouver, Wash.) Milwaukee Journal Sentinel Michael Hunt column
Source: Milwaukee Journal SentinelJuly 29--I've been around these parts long enough to have covered or seen Robin Yount's 3,000 hit, a couple of Packers Super Bowl titles, a handful of Rose Bowls, the Bucks' run within a game of the NBA Finals, two Brewers playoffs, Final Four appearances by Marquette and Wisconsin, the Mark Chmura trial and the divisive summer of Favre. A whole lot of sporting history has gone down in Wisconsin these last two decades, memorable, infamous and otherwise, but nothing has quite matched the pathos, the outrage, national impact and the historical significance of the rise and fall of Ryan Braun.Even with the passage of eight days to let some of the raw emotion subside, I'm still astounded at how strange, sad and precedent-setting Braun's case continues to be.After spending a weekend with the Brewers in Denver, I came away convinced that Braun confided his secret to none of his teammates. That suspicion was reflected in the measured, insightful words of Zack Greinke, who said the closer Braun got to anyone, the more he used them.It also became apparent that Braun's farewell address to his teammates a week ago Monday included no emotion, no apology, no explanation, just a see-ya next spring. How could that be? If there was ever a time for contrition, that would have been it. But again, Braun again chose arrogance over humility. What I saw during the first road trip after Braun's suspension was a hurt, saddened and betrayed clubhouse.Everyone thought they knew Braun when, in truth, no one knew the guy.Supposedly Braun's best friend, Packers quarterback Aaron Rodgers, had some insight into his restaurant partner. But Braun fooled him like he fooled everyone else. By the time Rodgers offered his candid and admirable words, he had to get in the back of a long line people Braun had cunningly misled. His owner, general manager, manager, teammates and fans believed him. At first I believed him. We迷你倉沙田were all played for suckers. Even then, the backlash against Braun would not have been as severe had he not thrown innocents under the bus during his sanctimonious, convincing spring training speech last year.No one wants to be made the fool, which leads us to the other remarkable, unprecedented byproduct of the Braun scandal.Never, at least in my memory, have other players so viciously turned on one of their own. MLB's rank-and-file's harsh criticism of Braun has been more widespread, pointed, severe and judgmental toward a peer than at maybe any other in baseball history. That, of course, is very good for Bud Selig's game. By making Braun an outcast, they have circled the wagons around the commissioner's get-tough drug policy. Finally, thanks in part to Braun, there is something approaching consensus on performance-enhancing drugs from the players and the union. That advancement alone makes the Braun story one of the biggest to ever come out of Wisconsin.Taking another positive from this lamentable tale, it was good to see how Rodgers freely and sincerely spoke his mind without being controlled by public-relations people.I had expected a PR guy to announce that Rodgers would only take football-related questions at the risk of ending his first training-camp interview. Instead, his comments on what seems to be his former pal were heartfelt and genuine and human, standing in stark contrast to Braun's cold and heartless words.Goodness knows Braun's high-powered Hollywood handlers have botched this situation, but maybe they, too, felt betrayed by their client.Could there have been one person Braun confided in during the last year and a half? To maybe say that, "Hey, I had these nagging injuries and we were in a pennant race"?If not, this story gets even weirder.Copyright: ___ (c)2013 the Milwaukee Journal Sentinel Visit the Milwaukee Journal Sentinel at .jsonline.com Distributed by MCT Information Services迷你倉價錢

Another death linked to Legionnaires' disease at Pittsburgh VA
Source: Pittsburgh Post-GazetteJuly 29--The death of a Delmont man in November 2011, after contracting Legionnaires' disease that was initially dismissed by the federal Centers of Disease Control and Prevention as not being part of the Pittsburgh Veterans Affairs outbreak, may have begun there after all, according to interviews and documents obtained by the Pittsburgh Post-Gazette.mini storageMost significantly, a spokesman at Forbes Regional Hospital said in a statement that the hospital is convinced that Frank "Sonny" Calcagno, 85, a Navy veteran who died Nov. 23, 2011, at Forbes, did not contract Legionnaires' there, as the CDC and the Pittsburgh VA believed."When looking at the full picture surrounding the case over time, however, including Forbes water test results and the lack of any other legionella cases, it is clear that this patient was not exposed to legionella at Forbes," spokesman Dan Laurent wrote in an email. The only other place Calcagno stayed in the weeks before he was diagnosed with the disease at Forbes was at the Pittsburgh VA.If that is the case, that would make Calcagno the sixth veteran to die shortly after contracting the disease during the VA outbreak in 2011 and 2012. After a 2-month-long investigation, the CDC in February determined that 22 men were sickened and five of them died during the outbreak at the Pittsburgh VA.But it found that another 11 veterans -- including Calcagno -- who had been patients at the Pittsburgh VA and had Legionnaires' at some point, contracted the waterborne disease outside of the Pittsburgh VA.The Pittsburgh VA's chief of staff, Ali Sonel, said the VA relied on the CDC's investigation -- which also involved staff from the Allegheny County Health Department and the Pennsylvania Department of Health -- in determining where all of the identified patients contracted Legionnaires'. He referred questions on that issue to the CDC.A CDC spokeswoman, Alison Patti, said officials at the CDC were too busy with other projects to comment this week.Calcagno's daughter, Debbie Balawejder, 58, of Monroeville, wasn't surprised to hear others say now that her dad contracted Legionnaires' at the Pittsburgh VA.She recalled that last November, after the Pittsburgh VA publicly revealed that it was dealing with an outbreak, "I was watching the news and when I heard about these people at the Pittsburgh VA dying from it, I turned to my husband and said, 'I know that's where he got it.' "Calcagno's longtime girlfriend, Becky Krezan, 75, who lived with him for the past 30 years, said no one at Forbes ever bothered to say where they thought Calcagno was infected.Calcagno's case appears to have been inadvertently dismissed as not being part of Pittsburgh VA outbreak because of a simple mistaken date.During a review of Calcagno's case, the CDC investigators determined that the date of "onset" -- that is, when he first began showing symptoms of Legionnaires' -- was on Nov. 5, 2011, according to the case review documents obtained by the Pittsburgh Post-Gazette from the VA through a Freedom of Information Act request. To determine the earliest possible date that a patient might have first contracted the disease, the CDC typically counts backward from the onset date by a full 10 days. But for this investigation, the CDC used a wider response time of 14 days.Counting back from Nov. 5 put the earliest possible date of infection on Oct. 22 -- the second full day that Calcagno was a patient at Forbes. But in another portion of the case review, the CDC investigator wrote that he was "admitted to Forbes Regional 10/21/2011 and + on 11/5/2011." That positive sign refers to Calcagno testing positive for Legionnaires' on that date.It would be unusual for a patient to show the first onset oself storage symptoms, be diagnosed, tested and have a test result come back the same day.The CDC's own list of the 32 possible Pittsburgh VA Legionnaires' cases shows that the time between onset and diagnosis -- which is typically the day a test result is returned -- can be as much as 10 days.Forbes would not provide a date that they believe Calcagno first began showing symptoms. His family says he began having a cough in the last days at the Pittsburgh VA, and it worsened after he got to Forbes on Oct. 20. (The Pittsburgh VA says he was admitted to Forbes on Oct. 21 but the family says he was admitted on Oct. 20, the same day he left the Pittsburgh VA because he had been having health problems. He had been at the VA since Aug. 28 -- during the beginning of the worst parts of the outbreak of Legionnaire's disease.)Sometime at about the end of his first week at Forbes, Mrs. Balawejder said she noticed her father's cough getting worse and that he had developed a fever, and she asked the doctors and nurses about it and was told: "Oh, that's just from his pneumonia."Legionnaires' is a potentially lethal form of pneumonia that is caused by the bacterium Legionella. It is transmitted by water, typically when a patient inhales the infected water into their lungs while showering or bathing. For younger, healthier people who contract it, Legionnaires' typically is not fatal. But for older, immune compromised patients like Calcagno, the CDC estimates that the percentage who die after contracting it can be as high as 30 percent -- which is about the percentage of Pittsburgh VA patients who died after contracting it there in 2011 and 2012.Experts say, however, that if the correct antibiotics are given to patients soon after infection, the disease can be cured, even in immune-compromised patients.Forbes says that Calcagno received antibiotics after being diagnosed with Legionnaires' and was in recovery when other issues worsened. But part of Mrs. Balawejder's and Ms. Krezan's anger is that they said they were never told that Calcagno had contracted Legionnaires' until a week before he died. Her father's doctor at Forbes, Palaniappan Muthappan, merely mentioned it one day as if Mrs. Balawejder had known for a while."I told him, 'No one told us that.' And he said, 'Well, it's on the chart,' like it had been there a long time," Mrs. Balawejder said. "Well, I don't read the chart."Her father had a series of other physical problems -- heart and liver problems, as well as thrush in his mouth and throat that made eating and drinking difficult -- that also contributed to his death. Before he entered the Pittsburgh VA on Aug. 28, 2011, because he had been feeling delirious from the anti-anxiety drugs, "he was fine," Mrs. Balawejder said."He was still driving himself, still paying his bills on his own, still working at Shop n' Save," she said, referring to his full-time job bagging groceries and stocking shelves at the Holiday Park grocery store. The Pittsburgh VA doctors helped treat some of his underlying problems, including fluid in his lungs, during a seven-week stay there. He made so much progress that near the end of his nearly two-month stay at the VA, Mrs. Balawejder said his doctor told her: "Your dad has at least five good years left in him."That was before he contracted the Legionnaires'.After he died, Dr. Muthappan ruled that the immediate cause of his death was respiratory failure, with underlying causes of pneumonia aspiration and renal failure. There is no mention of Legionnaires' on the death certificate.Sean D. Medicaid changes spur concerns locally
Source: The Columbian, Vancouver, Wash. July 29--In the next year, the state of Washington expects to have more than 600,000 newly insured residents.A significant portion -- about 330,000 people -- will receive medical coverage through the state's expanded Medicaid program. The Washington State Health Care Authority, which oversees the state's Medicaid programs, began work three years ago to implement the expanded program offered through President Barack Obama's Affordable Care Act.One year ago, expanded Medicaid was still considered a dream, said Manning Pellanda of the Health Care Authority."And now that we're less than three months out until we start, our vision is becoming a reality," he said.In Clark County, where about 60,000 residents are on Medicaid, that reality means an estimated 28,600 new Medicaid clients. It also means new concerns about the program's sustainability, the local physician workforce and the state's method of enrolling clients. As local physicians, medical groups and health plan administrators try to prepare for the patient surge, they're left with a number of unknowns."It's like building an airplane in the air," said Dr. Alfred Seekamp, chief medical officer for The Vancouver Clinic. "You don't know how it'll turn out, but you'll do your best."Expanding eligibilityUnder expanded Medicaid, people ages 19 to 65 making up to 138 percent of the federal poverty level will be eligible for benefits. For a single person, that's just under $16,000 per year. For a family of four, it's about $33,000 per year."This will be a big change," Pellanda said. "It's a positive change."The way people apply for Medicaid has also changed.Washington residents will apply for Medicaid benefits using the state's online health insurance marketplace, Washington Healthplanfinder. Open enrollment begins Oct. 1, but coverage won't go into effect until Jan. 1.Residents will go to the website and input their basic demographic information. From there, the system will connect to a new federal hub that stores information from the Internal Revenue Service, Social Security Administration and Department of Homeland Security. The system will also draw information from state offices, such as the Washington State Employment Security Department, which handles unemployment claims. That information will verify the person's citizenship (only U.S. citizens are eligible for Medicaid) and household income. The results will be displayed on the screen for the consumer to approve. Once the applicant clicks "OK," he or she will be added to Medicaid, Pellanda said."Instead of 45 days for an eligibility decision, you will find out in probably 45 minutes if you're eligible for Medicaid, which is fantastic," he said. "There's no waiting. There's no pending eligibility."Since the system relies on data that could be outdated, such as annual income tax returns, applicants have the opportunity to edit the information gathered through the data match to reflect their current circumstances, Pellanda said.If the person reports income below 138 percent of the poverty level, he or she will be declared eligible for Medicaid, he said. The state will run additional data searches afterward and will conduct random audits of about 20 percent of the total Medicaid population to ensure people aren't misrepresenting their circumstances, Pellanda said."Our data from many years experience is when you're thinking of people trying to fraud to get coverage, in the Medicaid world, we don't find that as a very large number," he said.Auto enrollment?New Medicaid clients will automatically be enrolled with a plan in their region. In Clark County, clients will be assigned to either Molina Healthcare of Washington or Community Health Plan of Washington, which contracts with Vancouver-based Columbia United Providers.Auto enrollment raises concerns for many in Clark County. If Clark County residents are assigned to a plan without a robust network of providers and specialists, they'll struggle to get access to care and will likely end up back in the emergency department, said Tom Sanchez, director of clinical operations at The Vancouver Clinic. Clients can request to be switched to a different plan, but the requests take time to work through the state department and result in delays in care, he said."That's happening now, and it'll continue to happen," Sanchez said.In the last year, health care providers and Columbia United Providers officials say Molina -- which only last year began managing Medicaid clients in Clark County -- hasn't signed up enough specialists to meet patients' needs. As a result, many patients are requesting to be switched to the Community Health Plan/Columbia United Providers plan.Molina Healthcare issued a statement saying it has been preparing for Medicaid expansion in Clark County and across the country since the Affordable Care Act was passed and is open to working with additional providers."In addition to expanding our network of providers, we are adding member services representatives to help members find primary care and specialty physicians as well as other social services they need to access health care and to stay healthy," the statement said. The Health Care Authority regularly evaluates network adequacy and is currently evaluating the Clark County plans, said Preston Cody, HCA deputy director. Based on his initial review, Cody said he's pleased with the results."Provider networks are growing," he said of the plans. "Their capacity continues to be able to serve Clark County."A small percentage of plan switching always occurs, and Cody said he hasn't noticed an unusual increase in Clark County. However, the state will continue to monitor plan adequacy, Cody said.Preparing for the 迷你倉價錢nfluxLocal officials believe many of the people who will be added to the state's Medicaid rolls are receiving medical care now, even though they're uninsured.Physicians are serving the uninsured at free clinics, in the emergency department of hospitals and in their own offices when insured patients lose medical coverage, said Ann Wheelock, Columbia United providers chief executive officer."I think it might not be the train wreck people think it will be," Wheelock said of the expansion. "These people are already getting some care."But it's unclear how many of those patients and other new Medicaid clients will be assigned to -- or will request to be switched to -- the Columbia United Providers plan. As such, providers who contract with Columbia, such as The Vancouver Clinic, don't know what to expect come Jan. 1.But Duane Lucas-Roberts, Vancouver Clinic's chief executive officer, thinks the clinic can handle an increased patient load. The Vancouver Clinic has more than 1 million visits to its medical offices each year. With 200,000 people seeking care at the clinic annually, even an additional 10,000 people would only be a 5 percent increase, Lucas-Roberts said."We think we have the capacity," he said.Still, the clinic has taken steps to prepare for the influx.Throughout the last year, The Vancouver Clinic has implemented several initiatives aimed at improving patient access, and by the end of the year, the clinic hopes to have hired an additional 10 to 15 physicians.Sea Mar Community Health Centers -- which provides medical, dental and behavioral health care to the insured and uninsured -- is also taking steps to prepare for more patients, said Mark Carlson, Vancouver Medical Clinic manager.Sea Mar is further developing its care teams by assigning case managers to people with chronic illnesses and hiring trained peer advocates -- many of whom have or have had their own chronic illnesses -- to work with patients, Carlson said.In addition, the clinic is recruiting more physicians and specialists. The clinic is hiring its own specialists since accessing specialty care in Clark County has been a challenge, Carlson said.Primary care concernsCarlson fears the expansion will exacerbate the specialty care access problems. Wheelock believes the access problem is bigger than just specialists."I think there's a big primary care access problem with all of health care reform," Wheelock said. The state Office of Financial Management issued a research brief in June 2012 about the availability of primary care physicians to serve the expanded Medicaid population. Nearly all urban areas in the state have enough capacity, according to the report. The one exception: Clark County. The researchers suspect a portion of new Clark County Medicaid patients will seek primary care in Portland. They based that conclusion on unpublished hospital discharge data that showed 19 percent of hospital stays by Clark County residents in 2010 occurred in Oregon hospitals. That won't likely happen, Wheelock said."We have for years tried to work with Portland providers," Wheelock said. "They've got Oregon Medicaid patients and no-pay patients and Medicare patients to take care of. They're not interested in caring for Washington's Medicaid patients."The state hopes increasing the Medicaid reimbursement rates will give providers incentive to take on more clients.Through the Affordable Care Act, the state is increasing rates for family practice, internal medicine and pediatric physicians. The reimbursement for a new adult patient's office visit will increase from $57.56 to $114.21. Reimbursement for an established adult patient's office visit will increase from $38.63 to $76.86.The higher rates went into effect this month, but the reimbursement is retroactive to Jan. 1. The federal funding for the enhanced rates expires Dec. 31, 2014.Wheelock questions what will happen once that federal funding is gone."If we're banking on federal money to save the day, I think we all need to be realistic," she said.Medicaid sustainabilityThe uncertainty surrounding reimbursement rates is only one concern local officials have about the future of the program. Many are also concerned about the sustainability of the state Medicaid program."Costs are going through the roof," Wheelock said. "This (expansion) is going to add pressure."Under the Affordable Care Act, the federal government will pay 100 percent of the cost to cover the newly eligible Medicaid enrollees for the first three years. After that, the federal contribution is scheduled to decline to 90 percent by 2020 and remain at that level. But many are skeptical the federal government will continue funding the program at those levels. Add in the state budget concerns, and the picture grows murkier."Somewhere there's going to be a point where all of this crashes," Wheelock said. "I think we're heading over a cliff."Moving forwardDespite the uncertainties, Wheelock is confident local providers will pull together and do their best to accommodate patients come Jan. 1."I think Clark County is probably as well-prepared as any other county in the state," she said.At the state level, the Health Care Authority is committed to make the new Medicaid program successful, Pellanda said."We do not want to see this fail in any way," he said. "And we will do whatever it takes to ensure the citizens of this state have access to care through the system we have put in place over the last three years."Marissa Harshman: 360-735-4546; twitter.com/col_health; facebook.com/reporterharshman; marissa.harshman@columbian.comCopyright: ___ (c)2013 The Columbian (Vancouver, Wash.)

Overweight military recruits a concern
Source: Chicago TribuneJuly 29--If you could've chosen any morning for a 2-mile run along the sparsely shaded path beside these railroad tracks, this one wouldn't have been it. It was only about 9:30, but the sun was already smothering everybody who stepped outside into the thick 90-degree heat.Herschel Gilbert, celebrating his 20th birthday, wasn't deterred. In a deep blue T-shirt that kept turning darker with sweat, Gilbert led three other young men on a jog north to the gas station, then back down the narrow trail to the parking lot where they had started. When they finished, Gilbert and the others dropped to the blacktop for a quick round of push-ups.At 5-foot-10 and a muscular 192 pounds -- he has since dropped to 184 -- Gilbert looked every bit the part of the Navy sailor he was training to become. He already peppered his speech with "Yes, sir" and could rattle off the Sailor's Creed as easily as he'd recite the alphabet. But when Gilbert first walked into the recruiter's office last December here in northwest suburban Crystal Lake, he had taken on the form of his occupation at the time: pizza delivery man. A trim football player during his days at Prairie Ridge High School, Gilbert ballooned to more than 250 pounds after graduation, making him much too large to enlist."Everybody knows about the freshman 15," Gilbert said. "I had the freshman 60."Americans' love of cheesy fries and aversion to exercise are both well documented. But while everyone has heard about the effect of high-calorie living on health care costs and kids' waistlines, scholars and former military leaders say the obesity epidemic also poses real -- but too often overlooked -- problems for national security.Gilbert, who this week heads to basic training at North Chicago's Naval Station Great Lakes, dropped more than 60 pounds by replacing the pizza with twice-daily workouts to meet the Navy's body-size requirements.Gilbert's story is exceptional in that he lost so much weight so quickly, but he's hardly alone in showing up to a recruiter's office with dozens of excess pounds. Almost a quarter of applicants to the military are turned away because they're too heavy, statistics show, and about three-quarters of young Americans aren't eligible to serve because of their weight or some other factor.Longtime standardIn 1887, with the Civil War still a relatively recent memory, the military introduced a chart prescribing an acceptable range of weights for various heights. The main goal was to weed out those who were too scrawny or sickly to serve.Today, recruiters face the opposite problem. Thousands of otherwise qualified young men and women are turned away -- or never bother turning up at all -- because they're too chunky."There are a lot more kids that come in just heavier," said Senior Chief Petty Officer Travis Elliott, who oversees six Chicago-area Navy recruiting stations. "It's one of those prevalent things that you see, and we just try to combat it as best as we can."In Gilbert's case, "combating it" meant an encouraging talk with his recruiter, information about potential workout partners and an invitation to check back weekly and use the recruiting station's scale. To enlist, he needed to either fall within an acceptable weight range or body-fat percentage range. The specific metrics differ slightly between the branches of service.By the end of February, Gilbert was still heavier than the 191 pounds allowed for future sailors of his height, but he was eligible to enlist because his percentage of body fat had dropped below the maximum level. He has since met the weight threshold, as well. There's a misconception, military officials say, that any 18-year-old with two legs and a high school diploma can join the service. In fact, recruits must pass a criminal history check, score well on a standardized academic test, not have certain chronic medical issues and meet the body-size requirements. Most young Americans, studies show, fall short of at least one of those standards. For now, enough people want to join the military that each branch is meeting its recruiting goals. But scholars warn that a rapid troop mobilization -- like those seen throughout U.S. history when the country prepares for war -- could result in too few qualified recruits. That's cause for concern, said retired Air Force Lt. Gen. Norman Seip."From a national security standpoint, you can have the best equipment in the world," Seip said, "but if you don't have the people to operate it, you just have a bunch of great static displays."Seip is part of a group of retired military leaders called Mission: Readiness. The organization, which publishes reports lamenting the number of ineligible young people, argues that government intervention to make school lunches healthier would help broaden the field of potential enlistees and make the country safer."By the time a young man or young woman shows up, 17 or 18 years old, if they've been living that unheal文件倉hy lifestyle, they're not going to turn it around in a month," Seip said. "We've got to put our resources against that epidemic."The weight restrictions are about much more than looking good in uniform. Command Sgt. Maj. Louis Barnum, who helps lead Chicago-area Army recruitment, said the rules are an important part of embracing a lifestyle of physical fitness that can save lives in a war zone."Having to shoot, move and communicate under fire, or having to carry our fallen off the battlefield, you've got to be in shape," said Barnum, who served two tours in Iraq. "When you're in life-and-death situations, you don't want someone slowing down."But having too few recruits isn't just an abstract concept: There's recent precedent. Just before the recession, with two Middle East wars raging and a stable private sector, the Army increased enlistment bonuses as it struggled to meet recruitment goals."There (were) a lot of casualties in Iraq and Afghanistan, and people weren't particularly eager to sign up. And the economy hadn't tanked, so people had other options," said John Cawley, a Cornell University researcher who co-authored an article in the journal Health Economics about the implications of rising obesity on recruitment.Enlistment numbers have since stabilized, but Cawley cautions that obesity rates could make speedy mobilizations a plus-sized challenge."You can't say it's currently crippling the U.S. military -- that's not true," Cawley said. "But it's a serious problem, and if we get in a situation that we have to rapidly expand the size of the military, then it's going to be a really seriously binding constraint."Military weight maximums have ticked upward as the American waistline has expanded, but further loosening of standards (and belts) could come at a cost. A 2004 study showed that almost 80 percent of recruits who initially flunked their weight test didn't complete their first term of enlistment. That comes at a significant expense to American taxpayers, who must then pay thousands of dollars to train another recruit. In 1991, the Army introduced a maximum body-fat percentage for people who couldn't meet the height-to-weight standards. Cawley and his co-author write that the adjustment provided an option for very muscular, in-shape young people to join. Those changes, one of several adjustments to the weight standards over the years, are a testament to how much American bodies have changed since the introduction of body-size standards.Recruiters are limited in what they can and will do to help prospective service members struggling with their weight. Barnum said recruiters are cautious not to give medical advice or prescribe a diet. If someone wants to shed fat, it's up to them to consult a medical professional and follow a responsible diet and workout plan."Our job is to find qualified applicants," said one Navy recruiter, Petty Officer 2nd Class Scott Rose. "It isn't so much to make qualified applicants."But that doesn't mean they write off everyone with pounds to lose. Rose was Gilbert's recruiter, and he said it was clear from their first conversation that the young pizza courier had the makings of a sailor."It was very evident that his interest was very sincere," Rose said. "He had nothing holding him back from joining the Navy except his weight. I hate seeing just one thing be a factor like that. Especially, in regards to fitness, it is something that can be corrected over time, healthily."That's the strategy Ethan Mueller, 18, employed as he worked to conform his 6-foot, 230-pound frame to Navy standards. In June, he met the body-fat percentage standards. He said he now weighs 200 pounds, a pound below the maximum for his height.Mueller attributes much of his weight loss to two things: cutting rocky road ice cream out of his diet and performing more manual labor on the Wisconsin dairy farm where he lives. He's scheduled to start basic training in North Chicago in September.Gilbert, who no longer delivers pizza, can empathize with the challenges of losing weight. After graduating from high school, he earned a basic medic certification from the City Colleges of Chicago with an eye on being a paramedic. But he had also replaced regular workouts with late-night meals of ramen noodles, mac and cheese, and assorted junk."I love cheeseburgers just as much as the next guy," he said. When he decided to enlist, he cut out the fattening meals and began working out. The pounds started dropping at a remarkable clip."I knew that I definitely needed to be in shape," Gilbert said. "I knew I was slacking a lot after high school and that really took a toll."I was just really motivated to kind of see where this would take me. So far, it's been a great ride."And that ride is just beginning. On Wednesday, Gilbert starts boot camp.mitsmith@tribune.comCopyright: ___ (c)2013 the Chicago Tribune Visit the Chicago Tribune at .chicagotribune.com Distributed by MCT Information Services存倉

Defense team rests case, Lepsch will not testify
Source: La Crosse Tribune, Wis. July 29--Jeffrey Lepsch's wife testified today that he isn't capable of executing two men and that he appeared normal hours after authorities believe he did.Angie Lepsch told jurors she found her husband at her in-law's La Crescent, Minn., home on Sept. 15 about 2 1/2 hours after the killings at May's Photo in downtown La Crosse."Did he appear to be excited?" Assistant State Public Defender Tom Huh asked."No," she said."Stressed?""No.""How did he appear?""Same as he always did.""Do you think Jeff is capable of murdering two people?"No."Four days of testimony in Lepsch's trial for killing store owner Paul Petras, 56, and his 19-year-old son A.J. Petras wrapped today with about an hour from the defense's only two witnesses, Lepsch's wife and mother.Neither explained what his attorney told jurors in his opening statement -- that Lepsch is "a thief and not a killer" who somehow ended up with 27 cameras, lenses and other merchandise worth about $17,000 stolen from the now shuttered store.Investigators found most of the items during an Oct. 10 raid at Lepsch's house in Dakota, Minn., and his van and searched his computer to find that he sold every other item online, according to testimony in La Crosse County Circuit Court.Angie Lepsch told jurors that her husband "bought and sold things on the Internet" to help support the family of five trying to survive on a preschool teacher's salary.A back injury sidelined him儲存for many jobs, so he absorbed most of the household work, Angie Lepsch said. She described him as a "very loving and affectionate" husband of 18 years who cared for animals and didn't miss the children's sporting events."Has he ever exhibited any kinds of violence?" Huh asked."No. Never," she said.At the time of the killings, the couple was behind on their mortgage and electric bill and had just $350 between two bank accounts, La Crosse police Lt. Matt Malott."As of Sept. 15 did he have enough money to buy $17,000 worth of equipment?" District Attorney Tim Gruenke asked."No," Malott said.Lepsch also was facing a $53,000 judgment levied against him from an 2003 felony theft case where he stole merchandise from the Ace Hardware distribution center in the town of Campbell over a 3 1/2 -year span while working in loss prevention.Angie Lepsch admitted on cross-examination that she didn't know about the theft until his arrest. She wasn't aware that he stored the stole merchandise from Ace at their home or that he was selling it online. She also testified she believed her husband owned just one camera and one or two lenses."You agree that he was able to keep a lot of things from you, correct?" Gruenke asked."Yes."Stay with .lacrossetribune.com for more from the trial.Copyright: ___ (c)2013 the La Crosse Tribune (La Crosse, Wis.) Drought, disease crimp farm outlook
Source: Bangkok Post, ThailandJuly 30--The Agriculture Ministry cut its growth forecast for Thailand's agricultural sector from 4% to a range of 1. 5% to 2.5% after only a slight expansion in the first half of the year. After the sector expanded by only 0.4% in the first six months, the ministry blamed the drop on the severe drought that hit widespread plantations from late last year until May, said ministry spokesman Apichart Jongskul.Early mortality syndrome (EMS), which ravaged Thai shrimp farming for several months and cut production by over 40%, was the main reason behind the drop in the fisheries sector.Other fisheries products were affected by the drought, causing the fisheries sector to contract by 7.1% year-on-year.Operators estimated revenue losses of 16 billion baht from falling shrimp shipments in the first half, a year-on-year decrease of about 30%.The export situation is expected to continue to decline due to the weak global ec迷你倉新蒲崗nomy, with the Thai Shrimp Association braced for a 40% fall in shipments this year.Crops were the only strong sector in the first half, with a 1.9% expansion from last year thanks to higher production of rice, cassava, oil palm, rubber, durian and mangosteen, said Mr Apichart.Besides proper irrigation that promotes rice output, the state's rice-pledging scheme has encouraged farmers to expand production and push the crop sector towards growth of 3.3% to 4.3%.The livestock sector also performed well, especially pigs, thanks to the reduced impact of porcine reproductive and respiratory syndrome.High demand for chicken meat in Europe and Japan will help shipments to rise by 5% this year. As of May, Thailand exported 219,000 tonnes of chicken products, up 4.7% year-on-year.Copyright: ___ (c)2013 the Bangkok Post (Bangkok, Thailand) Visit the Bangkok Post (Bangkok, Thailand) at .bangkokpost.com Distributed by MCT Information Services迷你倉出租
